Is there a way to assign restrictions to users' accounts on individual attachments?

The system is not specifically designed to do this.  When you logon to Millennium at the IIS you are aliased as the Anonymous Access User for access to any documents available directly from the Millennium interface, docs, pdf reports and html reports.  You can further restrict report access by using report groups. 
In Cloud Millennium (hosted) the exports are not accessible through the Millennium interface, but rather through the file system.  That is why users need AVHOST accounts to access  their export files through the SFTP site, files.avectra.com.  We do not capture your Windows logon account for use with Millennium.  Access to data is controlled by your Millennium logon so data access can be controlled by assignment to a group in Millennium.

 
There is an option to save the document in Word, Excel or other application with a password.  Here is how to put a password on the Word document.  When you do the save. select the Tools > General Options… from the Save or Save as dialog box.

When a user clicks on the link and select open, the document will download, select the document in Word and you see the following dialog box:
